Position Description

Summary

The principal function of the job is to provide care coordination/case management services in the home/office setting encompassing psychosocial, medical, educational, and functional assessment and intervention for children with disabling conditions. This is done to ensure that all children with disabling conditions reach their full potential through advocacy and education for the Family, thus improving the community's overall health.

 Responsibilities and Qualifications:

  • Provide case management/care coordination for recipients who have complex medical care and/or complex psychosocial issues.
  • Provide appropriate resources for the recipient and Family.
  • Provide monitoring and follow‐up per Plans of Care.
  • Resolve billing issues with families and providers.
  • Develop a pool of providers that provide services for the recipient and Family.

Qualifications:                                                                     

Education:  Bachelor of Science in Nursing (BSN) with a minimum of two years experience in Public Health Nursing, Maternal and Child Health, or Pediatric Clinical Care is required, or a Diploma in Nursing with a minimum of three years experience, or an Associate Degree in Nursing (ADN) with a minimum of four years' experience, or military equivalency. A current Michigan Nursing license is required.

 

Experience:  Nursing experience, including Family-centered care focusing on medically fragile children, is strongly preferred. Knowledge of health care and biological sciences with an emphasis on pediatrics is desired. Excellent interpersonal skills and written and oral communication skills are desired. Reliable transportation and a valid Michigan driver's license are required.
